Please only submit application once. It WILL go through! It may seem as though your application did not take, but it does. We have no way to correct this problem. You will normally hear from us within 48 hours as to the status of your application. "IF" we do not contact you within this 48 hour period, please e-mail us. We would love to hear from you. *I understand that this application in no way guarantees that a bird will be placed with me. I further understand that if I am approved I will be contacted if and when a bird becomes available or when my application nears the top of the list for type(s) of birds applied for. Approval is at the sole discretion of Pampered Parrots Rescue & Aviary.

*If you travel, who'd take care of your bird while your away? *Do you have an avian vet near by in case of emergency? *Which vet would you use if an emergency were to arise? *Add name & phone # of vet you would choose. *What is your prior parrot experience? Please describe in detail.
*It's important to provide a variety of foods to a parrot, what type of food would you feed?
*List some items that are toxic to parrots.
*Is this a Smoke Free Home? yes or no *Where will the cage be located? *Is this location in direct sun light or near air flow? * Do you understand that most rescue birds have behavior issues that will require extensive patience, consitence, love, work, and understanding to overcome? These issues may be so deeply ingrained in them they will never get over these behaviors. Will you be able to see past problems and love your parrot anyway? Be honest, and if there are some behaviors you can't tolerate please list them here so I can see to it we don't match you to a bird with this behavior. *Which behavior can you not tolerate? This does not dissqualify you, it just helps us to match you.
*Will you accept a parrot with behavior problems? *Will you accept a handicapped bird? *Have you ever been investigated or convicted of child or animal neglect or cruelty? *Do you give us permission to do a background check? We will need you to type your name and your Birth date and year to do this check.
*Explain why you would like to have a parrot.

We ask that you visit your bird at least once before it can go home with you. If you are more then 60 miles away, we will work out something with you. During your visit/s we ask that you stay at least 2 hours with your bird (this can be done over several visits or one). We will then set down with you and go over proper care for your bird along with answering any questions you might have. If you do not set up a visitaion date within a week of accepting your bird, your application will be denied.

*Do you agree to set up a visitation with your new bird within a week after a match is found and you have accepted the bird? Yes or No *Do you agree to send us a picture of your birds cage, toys and set up before picking him up? *It is mandatory at 6 months to send us a picture of your bird on it's cage. If we see a problem with weight change or feather condition that needs to be addressed, we want to help. Do you agree to comply with these terms? Yes or No I agree if Life changes keep me from caring for my new adopted parrot, I will return it to Pampered Parrots Rescue & Aviary. (Yes or No)
I agree if I can not tolerate the behavior problems my adopted parrot has or has aquired I will return it to Pampered Parrots Rescue & Aviary. (Yes or No)
*If for any reason I can not keep the Parrot I adopted, I will return it to the rescue. I agree I will not sale, rehome, or give away the bird to any other person / persons other then Pampered Parrots Rescue & Aviary. *Please type your signature and date if you agree to this contract and terms.
